​Please follow the instructions carefully. Make sure to call the clinic to make an appointment and have the information ready before completing the form. Voucher requests must be in our hands at least 5 days before the appointment date.  INCOMPLETE or late requests will not be processed.

We CONTINUE to provide Free to Low Cost to spay-neuter and vaccinate companion pets; vouchers are for residents of Gadsden, Liberty, and Jackson Co., FL.

If you Call 850.775.7781, we will take your information over the telephone; remember, we will still need a photo of the companion pet for which you are requesting this service.

Date of your appointment at the clinic and which clinic you will go to receive this service; please specify which service you are requesting.

you will pay your copay directly at the clinic!

OUR VOUCHERS ONLY COVER THE COST OF THE SPAY/NEUTER PROCEDURE, any additional services offered and accepted by you are not covered.
             
Veterinary clinics require your companion pet to be vaccinated against RABIES to perform this surgery. If your companion pet has not received this vaccine you can secure it at the same time you secure your spay, neuter voucher for an additional fee of $15.00. You will be saving at a minimum $10 by securing this service from our organization.

Spay or neuter your companion pet, talk to others about the importance of preventing animal overpopulation and stopping euthanasia!

             *The voucher(s) your are requesting must be for your personal companion pet(s). These vouchers are veterinary clinic(s) specific, you are responsible for transporting your pet to this location YOUR PERSONAL INFORMATION WILL NOT BE SHARED WITH ANY GOVERNMENT AGENCY AND WILL BE DESTROYED ONCE ALL THE VOUCHERS ARE ISSUED. 
                 

Is your dog or cat in need of being vaccinated? If the answer is NO we can help you make this possible. 
Yearly vaccines for dogs co-pay range from $0-$25 dollars
Yearly vaccines for cats co-pay range from  $0-$25 dollars
Yearly vaccines, heartworm and fecal test for both dogs and cats co-pay $40

If your dog/cat is sick, has suffered an accident, or doesn't look healthy and you are unable to afford the total cost of taking him/her to the veterinarian please give us a call 850.792.2152, we may be able to help you be able to afford the visit to the veterinarian. THIS SERVICE IS STRICTLY FOR GADSDEN CO. RESIDENTS.

YOU will have to provide us with a photo of you and your companion pet as well as proof of need prior to benefiting from this assistance. Please remember this is assistance we WILL NOT cover the total cost of any visit or intervention and we will only assist as long as funds are available.
